However this would be from prescription medicines, not illegal substances...and these are the ones we know about; there is apparently strong evidence that a large number of adverse reaction cases are never reported to the MHRA (Medicines and Healthcare regulatory authority).

or never found... most things at autopsy would not normally be tested for unless there is reason to run a test. and as most blood taken from a body at a standard autopsy is taken from the chest cavity many drugs can escape being found.
pretty much anything you take is going to react at some level with something else, its just a matter of how badly.
