This may have been posted before but I thought it was worth posting again.

This is a form which young people can take to A&E in the event of self harm and/or overdosing. Part is handed to the receptionist and the other part to the triage nurse - good if you don't want people overhearing the conversation or are struggling to find the words.

Used it myself and it made the whole ordeal slightly more bearable.

    Here's hoping.

    The staff weren't familiar with it but reacted well and it's actually the best treatment I've ever had.

    I'm amazed by how many NHS staff members aren't familiar with this. But they have all been very positive about it.
