Kind of all four together, really - I guess mostly it's been growing apart over time but sometimes that's been because of disagreements and/or moving away. I had a friendship that ended about a year ago, after having dwindled for quite a while due to her moving away, but the last straw was a disagreement between us. I think to her it felt like a betrayal, which I can understand  I tried to clarify that I hadn't meant to hurt her, but she never replied.
I know you fought hard as hell

    but let this sink in

you do not have to fight by yourself

I'd say a mixture of growing apart and ultimately different priorities in life

I went to a different secondary school to one of my best friends and even though we made the effort of trying to see each other regularly, eventually it just grew too difficult. However, I have come to terms with that and it's perfectly normal for friends to grow apart sometimes
