    GP should be able to certify you fit to go back to work, and you never know by now the stuff might have got through to your GP from the doctor. Failing that try ringing up the hospital and explaining, if they are treating you they should be able to certify you.

    i would have thought your GP could give you the sick note and fit to work note.
    As scary monster said, your GP should receive the info from the hospital to let them know what the diagnosis was/meds prescribed/follow-ups etc
