Fantastic - 5 fruit a day, no fast food. etc
Good - Generally healthy lifestyle, Brown bread, nothing processed.
Average - Fast food now and again, a lot of freezer meals etc
Poor - Fast food at least once a week, most meals processed/frozen.
Awful - Fast food several times a week, no fruit, no fibre.
I know those are sketchy but you can see what I'm getting at? Not sure if it can be quantified that way for a poll? I dunno..
Could follow it up with one about how many times a week people exercise though?
