    I had to print it off, so have no letter or envelope etc

    Just wanted to check are you talking about this Budgeting Loan? If so then your form may look like this?

    If it does then you need to take the completed form to your local Jobcentre Plus (part 12 of the form). To find your local Jobcentre Plus visit their website here.
