Question for you SpLDs out there, how much does it matter what I write on the form, and how much is based on the assessment? Explaining in writing on the form is going to be quite tricky, but I can articulate myself fine when desribing what the problems are.

Do you think I need to put loads of time into getting everything across on the form, or will the rough jist do. I have an Ed Psych report to go with it.

I think honesty is the best policy with this. Then they can figure out how best to help you.
Perhaps in this case, detailed is better... not so much feelings and stuff, just what the problems are and the difficulties they give you.

Oh, I was going to be completely honest, it's just a case of it's hard to describe without prompts.

I've actually got hold of the forms now, and in my case I hardly have to fill any of it in so should be fine.
