I seem to be having to say that too often today for my liking.
Secondly - it's usually best to create an actual survey rather than using the PM system. You've already invalidated any claims of anonimity by using a collection method that identifies the people who respond.
That may have a big effect on how many people actually take part. So you might want to take a look at something like Survey monkey to create something that can be taken online.
Also, and sorry to say it, but the terminology is pretty confusing. 'glorifying such things as guns, violence, knives, rape' - isn't in any way the same thing as a violent film. It may be best to actual say - Have you seen any TV shows that glorifies... etc. Otherwise you're really asking one question and then applying the answer to something else.
One last point - if you want people to consent to the idea that the survey 'shall only be used in the purpose of the study and shall not be revealed to any other participants, ' - then you'll need to define exactly what the survey is actually for, otherwise the consent is impossible. You can't agree to something only being used for it's purpose without knowing what the purpose is.
Anyway - maybe think about those points and let me know if you'd like anything changing in the survey (not if you'll be able to edit it in this part of the boards) - and I'll shift this over to 'We Need You' where it belongs.
